
Following the publication of the network of socio-health services in Elbasan, Korçë, and Shkodër, the Health for All Project (HAP) has now published the mapping of socio-health services to three additional municipalities: Durrës, Fier, and Lezhë.
In close collaboration with local civil society organisations, HAP continued its contribution to the integration of social and health services by providing mechanisms that promote better coordination in referring individuals who need support beyond the capacities of primary health care.
As in previous municipalities, the initiative has produced a detailed document titled “Network of Socio-Health Services”. A physical copy of this practical resource will soon be made available to all social workers operating within health centres in Durrës, Fier, and Lezhë, enabling them to guide patients toward the appropriate social services available in their communities, while an electronic copy is already available for download on HAP’s official website.
The mapping process, carried out from October 2024 to February 2025, involved consultations with a wide range of actors, including municipal structures, local NGOs, the Directorate of Social Protection and Inclusion, Regional State Social Services, and the Local Health Care Units. Their joint input has resulted in a comprehensive, area-specific overview of services addressing health, social care, psychosocial support, domestic violence response, child protection, and support for the elderly and people with disabilities.
By equipping frontline professionals with updated and localised referral tools, HAP is facilitating a more inclusive and efficient system of care, bridging the gap between health and social services, and helping communities thrive through greater access and collaboration.
